The information you give us will be used to inform our review and bring about change and improvement for the SAI process in the future. All information you give us will be treated confidentially.
We have three different surveys for specific staff groups, please could you read the information below to identify which survey you should complete:
If you have been asked to complete a survey by your organisation:
As part of this Review, we identified a number of SAIs which would be used as test cases. Staff associated with these SAIs should have been previously approached by someone in your organisation about completing a survey. If you have been approached previously and not yet completed a survey, we would be grateful if you could complete one of the following:
